By completing below, I [First Name] [Last Name] (Customer), authorize RELAY ENERGY BROKERAGE, LLC (Relay Energy) acting through its executives, administrators, or employees, to (i) access Customer’s account(s) with each Retail Electric Provider (“REP”) with whom Customer has an account; (ii) obtain copies of Customer’s payment history, billing history, establish and modify Customer’s payment information which is inclusive of but not limited to Customer’s credit card information and Automated Clearing House (ACH) information with each REP with whom Customer has an account; (iii) obtain copies of Customer’s historical usage information from Customer’s current or previous REP; (iv) communicate with a REP on behalf of Customer; and (v) negotiate one or more service agreements on behalf of Customer for retail electric service; and (vi) switch Customer from one REP to another REP on behalf of Customer. By its signature below, Customer hereby appoints RELAY ENERGY BROKERAGE, LLC as Customer’s agent with limited power of attorney to act on Customer’s behalf in obtaining the information described above and negotiating the agreement(s) described herein.
